Benefits for the period 10/1/2024 – 9/30/2025


Medical Insurance – United Health Care

We offer three group medical insurance plan options. If you choose this benefit, you will have to select one of these three plans. Details are available on the links below. The monthly premium rates are listed by age for each covered person.

Option 1: Balanced – CX-WQ / RX L27S – Benefit Summary

*CV-92 and CW-AR are the only medical insurance plans available outside of Florida

**Premium Rates are quoted monthly. We are paid bi-weekly. To convert the monthly rate to bi-weekly, multiply the rate by 12 then divide the product by 26. That will provide you with the bi-weekly Premium Rate.


Dental Insurance – United Health Care

We offer one group dental insurance plan. Details about coverage and cost are available on the links below.

Vision Insurance – United Health Care

This is a voluntary group insurance plan. Voluntary means that the employee pays the entire premium. There have been no changes to the vision plan coverages or premiums. Details are available on the links below.
